I, the parent or guardian of the registered child, do release and hold harmless The Crossing Church of the Nazarene and its representatives from any and all damages or liabilities arising out of sickness or accident occurred by my child.
I have listed any medical allergies, medications taken, food allergies, physical limitations or other pertinent information in my child’s registration information.
Permission: I understand that my child will be participating in activities at The Crossing Church of the Nazarene and will be under the care and guidance of its sponsors. I give my permission for my child to participate fully.
